jQuery mobile page

I'm in need of assistance with creating a page that uses jQuery mobile 1.4.5. The header, which includes the nav on this page seams to be working fine, however, the collapse and footer are not functioning as they should. I'm wondering if the template system tags are coded correctly, and requesting clarification? In addition, I need the page to be listed in the side nav of the current web page and don't see the "include in side navigation" anywhere. Please review both files attached and let me know where i'm going wrong, or if Cascade can even support this version of jQuery mobile? "LoanPageCopy.html is the original file that works as it should and "template-code" is obviously the template code that's being used for the loan page.

I created all new:
-Configuration Set
-Metadata set
-Data definition
-Content type

Stylesheets are in correct location and are referenced correctly in the head section. please let me know what I'm doing wrong...

    Hi Phillip,

    I reviewed your page. The issue that you were having was related to the misplacement of html tags.

    I went through and modified your template and your head block. Namely the following tag <div data-role="page"> needed to wrap the entire contents of the body of your HTML. This tag was in the header block and therefore only wrapping the header section of your page.

    I'd still like to spend 30 minutes chatting about best practices if you have availability on Monday afternoon. I am free at 2pm CT or after. Just let me know what works for you. I am closing this ticket out for now though.

    The modifications make sense. I appreciate you taking the time to look into this issue among the others….

    Next week is registration week and I have all day assignments on Monday. However, it looks like on Tuesday I’ll be in the office if you want to continue where we left off.
