Forms generator, plug-in, widget....anyone using one and embedding on Cascade Server web pages

We're looking for a tool to build forms that can be placed onto our webpages. We are currently doing something similar for a class registration page on our site, here is a link:

We don't want to write code each time we need to build a new user form. Any of you using such a tool for this? If so, what tool and would you recommend it to others?

    We have a similar interest. I actually just started looking this week. I know a few people on our campus are using wufoo.com. However, I would much prefer to have something in house that we control the data storage. 2 products that have popped up in my searches are:

    Machform - http://www.appnitro.com/
    Jotform Application - http://www.interlogy.com/products/jotform/

    I haven't had much time to look at them as of yet.

    Thanks Matt! I am copying Carrie Derks from our office. She is the person who is deciding on options for our forms builder.

    Other projects have taken priority over this and it has been put on the back burner fo now.
