workflows are coming in with changes that were not made

I've been having some unusual reports from some of my users the last few days. When users submit changes to the workflow, they appear to include edits that were not made by the users. for example, old versions of an image, or older versions of content. It's almost like its a previous workflow from the page that is getting resubmitted.

I'm running 6.7.1. Anyone have any ideas ?

    This might jive with a meeting I just had an hour ago with our information technology division. They, too, think old changes are somehow creeping back into pages, though we have no explanation for it or any way to prove it. We're on 6.7.4.

    I have a user who reviews and approves changes on the site. It's a 1 step workflow. I keep getting reports from her that when she approves the changes, somehow the content that gets published is an old version of the page. Anyone else experience anything like this. I'm having trouble replicating the error.

    I keep seeing the following errors for that particular user in the log files: 2010-11-17 15:31:36,592 ERROR [CascadeExceptionHandler] {User: mtice, id: 1c118e327f000001000568a83e05b3b2, type: page} An error occurred: null
    ClientAbortException: java.net.SocketException: Broken pipe

    Anyone have any ideas what might be causing this?
    I am attaching the last few days worth of log files.

    Are you able to attach your workflow XML here? I'm interested to see if it has a merge trigger in the step.

    I haven't been able to reproduce the behavior that your users mention seeing. Just to clarify - when these users report things like old images and old content appearing - are they referring to inside the CMS or when published?
